Apply investigative and learning processes, tools or methods to identify root causes and latent factors and to confirm corrective actions are in place that prevents SIF events

Apply a process that shares learnings related to the prevention of SIF events.

• Latent factors and context are recognized as critical for learning, such those related to culture, systems, processes, people and equipment. 

• Corrective actions address the root causes and latent factors and there is a process or method in place to confirm implementation.

• Sharing learnings with others in the organization strengthens our systems. 

• The Onshore Safety Alliance provides flexibility in how a participant meets the objective.
