Well Control Incident Level 2

No Loss of Well Control

Unintended influx or kick managed with a defined well control method but with complications.  No uncontrolled flow occurs.

Examples:

Type of Operation

Description

Drilling

While drilling ahead a pit gain is detected, the annular preventer closed, and the well is found to be flowing. While circulating gas from the wellbore, the annular rubber begins leaking but does not fail completely. A pipe ram is closed and the well kill is completed successfully.

Drilling

While circulating out an influx on a drilling rig, the choke hose ruptured and the rig loses the ability to monitor pressures and/or circulate. The choke line HCV was shut to isolate the leak and the hose was replaced.

Completion

During completion operations, surface pressure begins to rise in the wellbore. A kill/bleed off procedure is initiated with choke manifold valves incorrectly aligned resulting in unexpected flowback to a surface tank. The well is shut in and valve alignment is corrected.

Workover

During a slickline intervention on a live well, a leak in the pressure control equipment above the BOP is identified. The BOP is activated in an attempt to shear the slickline but shearing is unsuccessful. The actuated valve below the BOP on the tree performed as designed and sheared the wire. The well is secured, and failed pressure control equipment replaced.
